Important Information For CCDC Patients
Central Carolina Dermatology practices to obtain, verify, and record information that identifies each new patient. What this means for you: When you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We will also ask to see your driver’s license or other identifying documents.
MINORS: North Carolina law, G.S. 90-21.1, indicates a parent must give consent for any non-emergency treatment. Patients under the age of 18 must be accompanied by a parent.
We may experience inclement weather during the winter months. Our concern is with the safety of our patients and staff. In the event of inclement weather, please call our office the day of your appointment to confirm our office hours.
